2010 Reaffiliation Information for Environmental Groups
Please use the checklist to help organize your attachments. Please review every section thoroughly. Omission of any attachments could result in CFC denial or jeopardize your EarthShare affiliation.
Submitting your documents: Send all electronic documents in Microsoft Word or PDF format, to Aurora Oliva, aurora@earthshare-oregon.org. Attachments submitted electronically may be submitted in one document. Please label each item. Hardcopy documents: CFC application, ESOR agreement and all CFC and ESOR hardcopy attachments must be mailed or delivered to EarthShare Oregon by March 4, 2009 (no extensions). Submit in the order listed in the checklist in either a two-sided, pocketed folder or clipped together in separate bundles, one for CFC and one for ESOR.
